Information you give us. When you use our contact form, we collect your name, email, phone number, preferred way to reach you, and your message. Please don't include medical details in the form.

We use Google Analytics and our website host's built-in analytics to understand how people use our site. We do not use Google Analytics on booking, intake, or video visit pages.

Booking, intake, and visits
When you book, complete paperwork, or join a visit, you use secure outside services: Square (scheduling and payment), FormDr (intake and consent forms), and Kareo/Tebra (video visits). Each has its own privacy policy. Health information you share through these services is protected under HIPAA.
Text messages and calls
If you give us your phone number, we may call or text you to reply to you or to manage your care and appointments. Message and data rates may apply. Reply STOP to stop receiving texts.
Children
This website is not directed to children under 13, and we do not knowingly collect their information through it.
Security
We use reasonable safeguards to protect information. No website is completely secure, so please don't send health details through the contact form or by email.
